What prompted you to get involved with the further development of a tried-andtested, cement-free hip prosthesis based on the Zweymüller principle?

Dr Schneider: These hip prostheses have yielded excellent results in long-term studies. However, the development of atraumatic surgical techniques over the last 15 years has led to a need to modify our familiar, tried-andtested implants.

The anatomical short-stem shoulder prosthesis has only recently become available for clinical applications. There is still very little published data on the use of this type of prosthesis. The Australian shoulder expert Professor Bell discusses his experience.

Professor Bell, you have used the Affinis Total shoulder replacement for many years, and the results in the Australian Joint Registry are excellent.
What is your experiencewith it, and what motivated you to start implanting the short-stem prosthesis?

Vitamin E-stabilised vitamys has proven to be an extremely resilient and durable, highly cross-linked polyethylene for hip implants. How can its advantages be used to the full in knee endoprosthetics too?

The first RM Pressfit acetabular cup was implanted on 15th September 2009. To date, more than 11'000 of these implants have been used worldwide – with outstanding clinical results.

A success for doctors and patients.

A newly developed product is facing the toughest challenges imaginable: the preliminary results of a prospective, multi-centre study will show whether the  RM Pressfit vitamys cup, first implanted in 2009, is living up to expectations.

The first cross-linked polyethylene implants stabilised with vitamin E were used in clinical practice only a few years ago.

Professor Anract, why do you use the RM cup?
Thanks to its original fixation method, cemented anchoring is not required when a polyethylene cup is used without a ‘metal back’, i.e. without a metallic lining. «Metal back» cups with PE inserts have not impressed us so far due to the reports of premature implant failure. There have also been reports of worrying demineralisation of the bone in the area of the curved acetabular vault.

There are different types of cementless stems that use various anchoring systems available on the market. What do you feel are the advantages and limitations of the cone «philosophy»?
There are without doubt a great many different types of stems, with various anchoring systems. Some of them are metaphyseal, such as the Spotorno model, some are diaphyseal such as the Wagner model for revision surgery and some are metaphyseal-diaphyseal, such as the Zweymüller model.

Walking is a dynamic process. Yet to date, the functioning of artificial knee joints has been analysed using static images of the leg in straightened and angled positions. These are of limited effectiveness in resolving repeated incidences of artificial joints causing pain to some patients. And that is a problem, considering that around one million artificial knee joints are implanted each year, 40,000 of them in Switzerland alone. Hence the motivation for researchers at the Institute for Biomechanics to analyse the problem in closer detail (1). Mathys, a producer of artificial joints based in Bettlach, Switzerland, along with medical equipment manufacturer Philips, were enthusiastic about the idea and became involved in development work on a machine that could observe an artificial joint over the course of several walking steps.

Prof. Horne reports the results of his clinical study evaluating the radiological appearance for osteolysis of 131 titanium coated RM cups. After a mean follow up of 9.6 years he did not find any revisions due to aseptic loosening. In a subgroup of younger patients (< 60 years at operation) he did not find any radiographic evidence for pelvic osteolysis or loosening. Prof. Horne highlights the advantages of the RM cup (in terms of elasticity and reduced backside wear) compared to modular systems and advocates usage of monoblock cups.

100% survival for aseptic loosening of 102 RM classic cups after 13.4 years

Nagi ON, Kumar S, Aggarwal S. The uncemented isoelastic/isotitan total hip arthroplasty. A 10-15 years follow-up with bone mineral density evaluation. Acta Orthop Belg. 2006 Jan;72(1):55-64.

Prof. Nagi presents a clinical and radiological long-term follow up study with isoelastic total hip replacement and highlights the superior long-term results obtained with the RM classic cup. Within this study, he evaluated the differences of femoral bone mineral density (BMD) compared to the non operated side on 102 hips at a mean time after surgery of 13.4 years. He found in comparison to other studies very good results in relation to bone density around the proximal femur. The results are compromised by the high failure rate of the femoral component whereas no cup has been revised. No cup was found to be radiological loose and only minor ossifications (Brooker I) were detected in few cases (n = 7).

Many studies consider stem migration in the distal direction (i.e. subsidence) within the first two years as a predictive factor for early stem loosening.

The present study gives evidence that the Fullfix stem meets the requirements to avoid a stem subsidence within the cement mantle and shows excellent clinical results.
